What Are Community Health Coalitions?
Community health coalitions are community groups and individuals working with the Health Project in addressing a specific health improvement need identified through the Community Health Needs Assessment. These community-based coalitions are also known as collaboratives. The collaboratives develop specific programs to target at-risk populations with specific health needs. The Health Project collaboratives (see attached collaborative diagram) work to ensure that community resources are available to help those in need. In addition, they work as teams in the delivery of health services and education to the target population. They also assist in the data collection and outcomes reporting to the Health Project. The data gathered from these activities is used to measure the effectiveness of each collaborative in maximizing the limited community's health resources and improving the quality of life in the community..

What Are Hospital Coalitions?
Hospital coalitions are teams of skilled professionals based in the hospital who work with the Health Project to ensure that community values and health benefit needs are appropriately addressed. The focus of hospital coalitions may vary from addressing special health needs in the community like care coordination, to ensuring that the diverse community values are incorporated into the hospital culture itself.
